  • Chronic disease management such as Diabetes, Hypertension, Thyroid disease, and Anxiety / Depression and other.

  • Reviewing Test and / or Lab results.

What do I need for a Tele-Medicine visit?

  • Cell phone with a camera.

  • High speed internet connection.

  • Video conferencing app installed on your phone.

  • Please call us to receive the link for your video conferencing app and step-by- step instructions. 

What to expect in a Tele-Medicine visit?

  • Tele-Medicine visits can be scheduled by calling us at (503) 387-7111.

  • You will receive the link for your video conferencing app and step-by-step instructions for activation. 

  • Our staff will call you within 30 minutes prior to your scheduled appointment time to check you in and collect payment if applicable.

  • If it’s not covered by your insurance, our cash price for the visit is $165.

  • You will receive a video call from the doctor as soon as your check in process is complete.
